Application Form – Leave of Absence

Guidelines Please note that in all instances, the scholar must also obtain leave of absence from the tertiary academic institution. A leave of absence may be granted on the following grounds:

Medical or Psychological Illness as verified by a registered Medical Practitioner and subject to the opinion of the Chief Medical Officer where applicable. (The duration of such leave will
be guided by the prognosis for recovery.)
Pregnancy/Maternity for a period not exceeding one (1) calendar year
Optional Internship/Immersion Opportunities not exceeding one (1) academic year, as detailed in the "Policy on Requirements for Scholarships."
Bereavement or the Death of an immediate member of the scholar's family, that is, a parent (including parent by adoption), child, sibling, guardian, ward, spouse and cohabitant (in
accordance with the Cohabitational Relationships Act No. 30 of 1998) for a period not exceeding one (1) academic semester.
To facilitate a request to change the Institution of study or Course of study where such applications are pending for a period not exceeding one (1) academic year.
Where the scholar is seeking acceptance into an internship, Co-op or immersion programme that is a mandatory course requirement for continuation and/or completion of the Course of Study.
Where the scholar is unable to meet any additional costs associated with the Course of Study, not covered by the scholarship) for a maximum period of one (1) academic semester.
Any other situation that in the sole opinion of the Scholarships and Advanced Training Division (SATD) is both extenuating and beyond the control of the scholar.

NB: All leave of absence from the course of study must be approved in writing by the Ministry prior to the scholar proceeding on leave.

>Official Medical Certificate/Report from a Registered Medical Practitioner Medical Certificates must state;

The duration of the illness/pregnancy with the proposed due date

>Letter from the Academic Institution recommending and giving approval to the scholar to pursue the optional Internship/Immersion Opportunity

Duration of the Immersion/Internship
Obligations of the scholar (if any)
